Consent to personal data processing
Consent to personal data processing of individuals using feedback form:
Hereby, the user sending a message through feedback form on www.profmedassist.com accepts the Consent to personal data processing (hereinafter referred to as Consent). The message sent through feedback form is considered as the acceptance of the Consent. Acting by his/her own free will and in his/her own interest, as well as confirming his/her active legal capacity, the User grants his/her consent to personal data processing by JSC ‘International Medical Clinic’, registered at 16 Dokukina street, building 3, premises 2, room 28, 129226, Moscow, Rostokino district, on the following conditions:1. The Consent is given for personal data processing either with or without the use of automation.
2. The Consent is given for processing of personal data which are not special or biometric: name, surname, phone number, e-mail.
3. The personal data shall not be treated as the public data.
4. The purpose of processing personal data: providing responses to requests, return calls on feedback form requests.
5. Personal data processing is based on article 24 of the Constitution of the Russian Federation, article 6 of Federal law ‘On personal data’ as of July, 27th 2006, No 152-FZ, the charter of JSC ‘International Medical Clinic’.
6. The processing of personal data means the following list of actions with personal data: collection; systematization; accumulation; storage; clarification; usage; blocking; destruction.
7. The following third parties may process the provided personal data on behalf of JSC ‘International Medical Clinic’: JSC Profmedassist Sakhalin, registered at 7/1 Militseyskaya street, 693020, Yuzhno-Sakhalinsk.
8. Personal data are processed before a public offer is terminated. Also, the processing of personal data may be terminated at the request of the personal data subject.
Storage of personal data recorded on paper is carried out in accordance with Federal Law ‘On Archives in the Russian Federation’, No 125-FZ, and other regulatory and legal provisions in the field of archiving and archival storage.
9. Consent is given, among other things, to a possible cross-border transfer of personal data to countries, which do not ensure adequate protection of the rights of personal data subjects.
10. Consent may be withdrawn by the subject of personal data or his authorized representative by sending a written application to JSC ‘International Medical Clinic’ or its representative at the address indicated at the beginning of this Consent.
11. In case when the subject withdraws consent to the processing of personal data, JSC ‘International Medical Clinic’ may proceed with processing personal data without a subject's consent, if there are grounds stipulated by paragraphs 2-11 of article 6, part 1, by article 10, part 2, and article 11, part 2, of Federal Law ‘On personal data’ as of July, 27th 2006, No 152-FZ.
12. This Consent is effective until the processing of the subject's personal data is terminated on the grounds set out in paragraphs 8 and 10 of this Consent.
